Fourth Street Clinic helps individuals experiencing homelessness improve their health and quality of life by providing high quality integrated health care.
Fourth Street Clinic, located in Salt Lake City, Utah, is a Federally Qualified Health Center (FQHC) funded through the Health Resources and Services Administration, of the United States Department of Health and Human Services. Fourth Street Clinic is designated as a homeless health center which mandates that at least 75% of our patients are homeless.
Founded in 1988, Fourth Street Clinic was a response to the unmet medical needs of the growing homeless population in Utah. Initially operating as a one-room triage clinic with a part-time nurse, the clinic relied on local hospitals for patient treatment. In 1993, Fourth Street Clinic relocated to its current facility at 409 W 400 S, Salt Lake City, and attained nonprofit status in 1998.
Since then, the clinic has expanded its services to enhance integrated, comprehensive care for this underserved population. Significant milestones include the opening of an onsite pharmacy in 2001, the introduction of behavioral health services in 2011, and the launch of an onsite dental clinic in 2014. In 2019, Fourth Street Clinic further broadened its reach by introducing a Mobile Medical Clinic and expanding the Medical Outreach Street Team, reinforcing its commitment to providing accessible, low-barrier health care. Today, Fourth Street Clinic employs over 130 dedicated professionals and operates with an annual cash budget of $18 million.
Over the past 37 years Fourth Street Clinic has exemplified its mission through addressing the health disparities, and complex needs of individuals and families experiencing homelessness by providing comprehensive medical, behavioral health, dental and other enabling services through an integrated care model utilizing the principles of Trauma Informed Care. In 2024, we served 6,606 unique individuals, including children—96% of whom were currently experiencing homelessness—through 30,284 patient visits. This is a 12% increase over the previous year. Our pharmacy played a critical role in this care, dispensing 93,517 prescriptions last year.
As a Federally qualified Health Center, Fourth Street Clinic serves patients regardless of their ability to pay. Health insurance is accepted but not required. Fees are based on a sliding scale tied to the Federal Poverty Level (FPL), with no charge for those below 100% FPL and a $1–$4 fee for those between 100% and 200% FPL. In 2024, 88% of patients had incomes below 100% FPL.
